Practical Request Form

In order to better assist firefighters, coordinators and proctors, we have standardized our practical requests to an e-mail system. Practical requests should be emailed to Marlinda Acevedo.

We require that all practical requests are received by the Division at least 30 days prior to the test. Regretfully, we cannot accept phone calls for request practical exams.

IMPORTANT: In order to process your request, please copy, paste, and complete the following information in the subject line of your email:

In the subject line of your email, please include: Practical Request: [Name of Dept./Distr. or Organization] / [Type of Exam] / [Date of Practical]

Please copy, paste and answer the following information to be included the body of your email:

  1. Name of Department/District/Organization Requesting Practical:
  2. Practical Date:
  3. Practical Time:
  4. Number of Participants:
  5. Location of Practical:
  6. Practical Level (i.e. FFI, HM Ops, etc.) :
  7. If Firefighter I Level, is this a request for a Live Burn?: Yes/No
  8. Lead Proctor's Name:
  9. Lead Proctor's Number (i.e. M0001):
  10. Lead Proctor's Email Address (Separate multiple addresses with comma. This is REQUIRED!):
  11. Are Personnel not affiliated with the sponsoring fire department allowed to participate?: Yes/No
  12. Name of Coordinator/Contact:
  13. Telephone Number of Coordinator/Contact:
  14. Email Address of Coordinator/Contact:

Please be advised that the state certified lead proctor should receive the emailed JPRs no later than 72 hours prior to the evaluation.

Practical Exam Information

Pursuant to the Colorado Division of Fire Safety Practical Examination Proctor Guidelines: Proctors and Assistant Proctors shall not be used as evaluators
for skill test areas that they have instructed. Proctors and Assistant Proctors are required to hold a valid Colorado certification at or above the level of practical examination they are proctoring.
